Develops and maintains partnerships with community service providers and health professionals. Works with designated staff to the develop programs, maintain records, delivers program evaluation data, completes required reporting, and ensures adherence to BCBS grant requirements. Under indirect supervision, works closely with medical providers, primary care teams, and social services agencies to provide short term care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings.
Minimum Job Requirements:
High school diploma or equivalent, required;
1-2 years minimum Health related Field, preferred;
Successful completion of Community Health Worker certification program within six months of hire date.
