PRN - As Needed
Performs general personal care procedures and associated homemaker functions for patients under direction of the Registered Nurse and as defined in the established Home Health Aide Care Plan. Works in conjunction with all disciplines a team to provide quality patient care.
Ability to perform the duties and responsibilities of this position, with or without reasonable
accommodations for disabilities.
Majority of work is done in patient's home with potential for exposure to contagious illnesses.
Irregular hours required. Personal automobile used for travel.
Knowledge, Skills and Abilities:
Knowledge of proper body mechanics and basic personal care procedures.
Knowledge of observations required for patient condition.
Skill in performing personal care tasks, observing patient condition.
Skill in oral and written communication and interpersonal relations.
Skill in patient teaching techniques.
Ability to report changes to RN.
Ability to be flexible and responsive to the needs of patient and family.
Ability to successfully complete a written and skills competency evaluation test.
Ability to live the values of Shannon Medical Center.
Ability to adhere to hospital policies and procedures governing patient privacy, appropriate access and confidentiality of patient information.
Education and/or Experience:
Ability to read, write and speak English, bilingual preferred. Minimum of six months experience in direct patient care giving personal care in an institutional setting within the past five years. Home Health experience preferred.
License, Certification of Registration:
Certified as Certified Nursing Assistant required.
